Cover page and back for our asbestos survey reports

Need a professional and eye catching cover page for our asbestos survey reports. At the moment is is quite standard, just showing logo and table for a few details.

I've attached current front page and also an example from another company.

I'd Like something that takes up the full page with table to add client data and photo of building - must be Word compatible.
I'd like something high definition and eye catching.
