I need an online booking confirmation template

We run a taxi and minibus booking website and the website is capable of sending auto confirmation to the client however when a client makes a booking over the phone we have to manually create a booking confirmation and an invoice to be sent out to the client. We want a tool that can do the following for me.

A template for online booking confirmation where we can simply input the details and send it to customers by email.
This should be compatible with desktop PC, tablets and mobile phones.
The fields that we want in the booking confirmation are.

Always make sure that you get in the right vehicle by confirming your name and destination with the driver.
In case of pick up from an airport, please ring us on the number provided as soon as you have landed and make sure that your mobile phone is switched on so the driver can contact you.
