Create PDF invoice template using FPDF PHP class

I am currently working on a large project for a UK client to construct a PHP-based CRM and sales order management system.

I need somebody who is familiar with the FPDF PHP class to output a single A4 PDF page to the client's design and specification. The page will consist typical elements such as a common page header which includes image logo, company name, address, other contact details (tel, website, email), VAT number. The page body will contain customer details (name, address, contact name, purchase order number), delivery address, and there will be a common footer containing information including strapline and a couple of product certification images, plus the details of the the sale itself (line number, stock code, product description, quantity, price each, prce total, etc). The first dummy line of the invoice will need to be inserted, and I will ultimately put this line into a loop so all of the correct invoice lines can be inserted.

This is a small mundane project for somebody who is familiar with FPDF to position the various cells correctly on the page. I will supply the final design in the form of a Word DOC for the successful bidder to obtain all of the element positions on the page. For delivery, I shall require a single PHP script to add the page, position the body elements, and output the page, plus the common header and footer functions.
